Hounslow Heart Failure Patient Monitoring

Compared to England averages, Hounslow suffers from poor rates of heart failure recording and high rates of non-elective emergency admissions. We have recently agreed to support a one year pilot project based at the Cardiology Department of our West Middlesex Hospital site which aims to improve care for these patients. by implementing change at four levels:

  • Promote best practice in existing hospital patient pathways
  • Deliver a new heart failure ambulatory clinic
  • Identify patients receiving suboptimal care in the community
  • Empower patient self-care and monitoring at home through novel m-health solutions

The project aims to reach 2,000 patients and 52 primary care practices in Hounslow, by delivering a holistic, integrated and technologically enriched approach to heart failure care best practice. Based on previous work by the Cardiology team, the project aims to reduce heart failure non-elective bed days at West Middlesex by 15%, equating to an expected cost saving of approx. £200,000 per annum.

The technology will be provided by Medopad, a London Digital Health.London Accelerator company.
